Intensive Case Management

The Intensive Case Management stream is for families who require intensive support to keep their family strong and children safe.

This stream prioritises Aboriginal families and children.

Referrals are made by the Department of Communities.

These families are likely to experience longer term challenges in life that they may require additional support. The lead agency will be flexible and adaptable to the families’ needs.

The lead Agency will actively engage with families to achieve family and case plan goals involving in-home practical ‘hands on’ support for up to 12 months.
